Description: Diabetes affects approximately 23.5 million adults in the United States, including the vulnerable subset of elderly patients who are residents of long-term care facilities. This quality improvement initiative will address the challenges of effectively managing these patients, recent developments in diabetes management, systemic barriers to providing optimal care, and coordination of care with outside healthcare professionals, with the ultimate goal of making clinically significant improvements in patient care.
